Chronic Disease Management
Monday evening are reserved for the Chronic Disease Management (CDM) program which provides comprehensive treatment for hypertension, diabetes and other endocrine disorders.
In addition to nurse and physician consultations, CDM patients receive nutrition counseling, ophthalmology consults, diabetic podiatric exams, routine lab testing as well as medical supplies and medications. CDM visits are by appointment and for enrolled patients. To enroll, patients must first be seen during a Tuesday or Thursday walk-in Clinic for one of the chronic illnesses listed and be referred into the CDM Clinic. |
Patient Eligibility Requirements
Chronic Disease Management (CDM) services are by appointment only. Patients must be referred to this program after becoming our patient.

- Have no form of health insurance or health care coverage (i.e. no MAP, Medicaid/CHIP, private insurance, etc.)
- Meet income guidelines (less than 200% of the Federal Poverty Income Level)
- Present a Photo ID
- Present Proof of current Travis County address (ID, utility bill, lease agreement, etc.)
