Max with kids

DESCRIPTION: Provides non-emergency medical treatment for children and adults who have low income, and have no other access to health care. Services include: primary care services for acute and chronic conditions, laboratory work and x-rays (referrals only), referrals to medical specialists and other medical and dental resources, and medication assistance. Provides a Chronic Disease Clinic for diabetes, gynecology, and hypertension by referral only from the Public Clinic on Tuesday and Thursday evenings.

HOURS: Tues and Thur 6:00pm - 10:00pm (phone not answered during clinic hours)

INTAKE PROCEDURE: Line up outside the clinic at 5:30pm on Tuesdays and Thursdays; 35 adults and 15 children will be given numbers to be seen

SERVICES NOT PROVIDED:The Volunteer Healthcare Clinic does not provide emergency care, pregnancy testing/prenatal care, immunizations, routine physicals, STDs, mental health services, dental care, or pap smears/routine GYN services. FEES: $5.00 donation requested

ELIGIBILITY: Low income adults and children without health insurance (CHIP, MAP, Medicare, etc.)

DOCUMENTS REQUIRED: Photo ID (other than passport); proof of income, such as recent pay stubs or tax forms
